“Pickleball elbow” isn’t an officially recognised medical term, but if you’ve played the sport for any significant time, you’ve either experienced it or heard someone complaining about it.
It’s a genuine and common injury. It sidelines players. And a surprising amount of it is preventable with the right gear and technique. This is what you need to know.
What Is Pickleball Elbow?
“Pickleball elbow” refers to lateral epicondylitis, the same condition commonly called tennis elbow, occurring in the context of pickleball play. It’s an overuse injury affecting the tendons that attach to the outside of the elbow.
The symptoms:
- Pain on the outer side of the elbow, particularly when gripping or extending the wrist
- Tenderness to touch along the outside of the forearm near the elbow
- Pain when lifting or gripping objects (coffee cups are a classic early warning sign)
- Stiffness in the morning, improving with movement
Left unaddressed, lateral epicondylitis can become a chronic condition that takes months to resolve. The sooner you identify it and adjust, the faster the recovery.
What Causes It in Pickleball?
Several factors specific to pickleball contribute to elbow injury:
Volume of play
Pickleball’s social format makes it easy to play for much longer than you intend. “One more game” becomes three more, becomes a two-hour session. Tendons don’t fatigue the way muscles do; they don’t give you the same clear warning signals. You feel fine until you don’t.
Players new to the sport are particularly at risk. Enthusiasm outpaces the body’s adaptation. If you’ve just discovered pickleball and you’re playing every day, which I completely understand, be deliberate about rest days in your first few months.
Hard, stiff paddles
This is the gear factor that most players underestimate.
A paddle that’s too stiff (thin core, hard face) transmits vibration directly into your arm on every contact. In a long session, you might hit the ball 300-500 times. Each contact sends a vibration up your arm. Over hundreds of contacts, a stiff paddle creates cumulative micro-stress on the elbow tendons.
The difference between a 16mm EPP foam core and a 12mm fibreglass paddle in terms of vibration transmission is significant. Thicker, softer cores absorb impact better and transmit less shock to the arm.
Over-gripping
Players who grip the paddle too tightly, white-knuckled through every shot, activate the forearm muscles more than necessary. Over time, this creates the same overuse pattern that causes elbow injury in tennis players.
The correct grip pressure is loose. Comfortable. Especially when dinking and rallying. You shouldn’t be squeezing the handle through exchanges.
Poor technique on the backhand
The backhand is the shot most associated with pickleball/tennis elbow. Specifically: hitting the ball late (behind the body) and compensating with the arm rather than the shoulder and core. This puts enormous rotational stress on the elbow tendons.
Hitting the backhand with proper forward contact (ball in front of the body, shoulder driving the stroke) dramatically reduces this stress.
How to Prevent It
Warm up properly
Five minutes of light movement before playing (arm circles, wrist rotations, gentle forearm stretches) primes the tendons for load. Cold tendons under sudden stress are more vulnerable than warmed ones.
This isn’t dramatic. It’s just 5 minutes before you pick up the paddle. Make it a habit.
Manage your session length
If you’re new to the sport, cap your sessions at 60-90 minutes for the first few months. If you feel any elbow discomfort during a session, stop. Tendons take much longer to recover from injury than muscles do. A mild warning sign ignored becomes a three-month lay-off.
Work on your backhand technique
Get your contact point right. Ball in front, shoulder driving, wrist stable at contact. If you’re unsure about your technique, watch a few instructional videos focused specifically on backhand mechanics; there’s good coaching content available online.
Choose the right paddle
Weight: Mid-weight paddles (7.5-8.5 oz) are generally lower-risk than heavy paddles. Very heavy paddles (8.5 oz+) put more load on the forearm with every swing. Very light paddles (under 7 oz) can cause over-swinging to compensate for the lack of mass.
Core thickness: Thicker cores (14-16mm) absorb more impact and transmit less vibration to the arm. If you’re already experiencing elbow issues, a thicker-core paddle is a meaningful change.
Face material: Composite and carbon fibre paddles with proper cores are generally better for arm health than hard fibreglass paddles that transmit vibration directly.

If You’re Already Injured
Rest first. This is genuinely the most important thing. Continuing to play through elbow pain makes the injury worse and extends recovery time significantly.
Ice and anti-inflammatory: Applying ice to the affected area after activity and taking anti-inflammatory medication (where appropriate) can help manage the acute phase. Consult a doctor or physiotherapist if the pain is significant.
Physiotherapy: A physiotherapist can diagnose the severity, provide targeted exercises to restore tendon strength, and identify any technique or equipment issues contributing to the injury. For persistent or severe cases, this is the right path.
Gradual return: When you return to play after an elbow injury, start conservatively: shorter sessions, lower intensity, and pay attention to any returning pain. Don’t rush back to full volume.
Preventing It Comes Down to a Few Habits
Pickleball elbow is common but largely preventable. The combination of proper warm-up, session length management, good backhand technique, and the right paddle spec significantly reduces your risk of developing the injury in the first place.
Gear matters here. A paddle that’s too stiff, too heavy, or too hard transmits more impact to your arm. Choosing a paddle with a proper core thickness isn’t just a performance decision. It’s an injury prevention one.
